Mint Hill is on the Monroe side of Charlotte, so the drive to the clinic is shorter than the drive to our old office was: twenty-five minutes down NC-51 and US-74, with the certificate in hand the same day.

About 15 miles, about 25 minutes. From Mint Hill take NC-51 south to US-74 (Independence Blvd) and head east, or Lawyers Road east to NC-218 and on to US-74. Stay on US-74 Business (Roosevelt Blvd) into Monroe; the clinic is on W Roosevelt Blvd, inside Keith Clinic at Suite A.
Mint Hill and the Albemarle Road corridor have a lot of construction, dump and concrete drivers whose cards come up for renewal in the middle of a job. Saturday hours mean you do not lose a working day to the exam.

The price
$79 cash
or $81.77 by card
Covers the exam, the urine test, your certificate and the National Registry submission. Nothing else to pay.
Arrive ten minutes early for paperwork. Dr. Tebby checks vision, hearing, blood pressure and pulse, runs the urine test (a medical screen, not a drug test), goes through your history and medications, and does the physical. Most drivers are finished in 30 to 45 minutes. If you meet the FMCSA standards, your certificate is signed and printed before you leave and submitted to the National Registry the same day. The Registry passes the result to the NC DMV electronically; you no longer hand in the paper certificate.
Bring your CDL or photo ID, a written list of your medications and doses, glasses or contacts if you drive with them, hearing aids if you use them, your current card if you are recertifying, and your CPAP compliance report or a specialist’s letter if either applies to you.
How far is it from Mint Hill?
About 15 miles, about 25 minutes by road, and the Google Maps link above gives you the live route and traffic. Truck parking is on site.
Does the exam include a drug test?
No. The DOT physical includes a urine test that checks for medical conditions, not drugs. If your employer needs a DOT drug screen as well, that is a separate collection we arrange through a partner lab; see our DOT drug testing page or ask Alex when you book.
What does it cost?
$79 cash or $81.77 by card, and that is everything: the examination, the urine test, the certificate and the National Registry filing. If you were already booked at a quoted price, you pay the price you were quoted.
