SMS Appointment Reminder Program

Charlotte DOT Exam Center operates an automated SMS appointment
reminder program for patients who have undergone a DOT physical
examination at our facility.

How we collect your phone number: Patients voluntarily provide
their cell phone number to our staff at the time of their DOT
physical examination or when booking an appointment by phone.
Consent is obtained in writing via a signed paper consent form
at the clinic, or verbally via our AI phone booking assistant.
Participation in the SMS reminder program is completely optional
and is not required to receive DOT physical examinations or any
other services at Charlotte DOT Exam Center.

How we use your phone number: We use your phone number solely
to send automated text message reminders notifying you when your
DOT medical certificate is approaching its expiration date.
Messages are sent at approximately 30, 15, 7, and 1 day before
expiration.

Message frequency: Up to 4 messages per certificate cycle.
Message and data rates may apply.

Opt-out: Reply STOP to any message to unsubscribe. You will
receive no further messages. Help: Reply HELP for assistance or
call us at (704) 544-3494.

Data sharing: Text messaging originator opt-in data and consent
will not be shared with any third parties. Charlotte DOT Exam
Center does not sell, rent, lease, or share patient phone
numbers, SMS opt-in data, or consent records with any third
party for any purpose, including but not limited to marketing,
advertising, affiliates, analytics providers, or data brokers.
Phone numbers are used exclusively for the DOT Medical
Certificate Expiration Reminder Program as consented.

Retention: SMS consent records are retained for a minimum of
three (3) years consistent with applicable medical record
retention requirements.
