DOT Physicals for Ballantyne, NC Drivers
$79 DOT physical, about 40 minutes from Ballantyne
Ballantyne to the Monroe clinic is forty minutes, and the Providence Road route through Waxhaw avoids the interstate altogether. One Friday or Saturday visit, one card, and you are back in south Charlotte by early afternoon.
Certifying commercial drivers since 1985. Alex books 24 hours a day in English and Spanish and texts you the address as soon as you are booked. Walk-ins welcome during clinic hours.

Getting to the clinic from Ballantyne
About 30 miles, about 40 minutes. From Ballantyne take Providence Road (NC-16) south to Waxhaw, then NC-75 (Waxhaw Highway) east into Monroe, or I-485 north to US-74 east. Either way the clinic is on W Roosevelt Blvd, which is US-74, inside Keith Clinic at Suite A.
Ballantyne is corporate Charlotte, and the drivers we see from there are as likely to drive a charter coach, a courier van or a company straight truck as a tractor-trailer. All of them need the same FMCSA certificate, and we examine every driver type at the same price.
Related: charter and tour bus drivers, non-CDL commercial drivers, owner-operators.
Where the exam is held right now
Charlotte DOT Exam Center has certified commercial drivers since 1985. While our new Charlotte facility is finalised, Dr. Alan Tebby, one of our two FMCSA-certified medical examiners, holds clinic in Monroe on Fridays and Saturdays, inside an established medical practice with truck parking on site.

Covers the exam, the urine test, your certificate and the National Registry submission. Nothing else to pay.
What happens at the exam
Arrive ten minutes early for paperwork. Dr. Tebby checks vision, hearing, blood pressure and pulse, runs the urine test (a medical screen, not a drug test), goes through your history and medications, and does the physical. Most drivers are finished in 30 to 45 minutes. If you meet the FMCSA standards, your certificate is signed and printed before you leave and submitted to the National Registry the same day. The Registry passes the result to the NC DMV electronically; you no longer hand in the paper certificate.
Bring your CDL or photo ID, a written list of your medications and doses, glasses or contacts if you drive with them, hearing aids if you use them, your current card if you are recertifying, and your CPAP compliance report or a specialist’s letter if either applies to you.
Ballantyne drivers ask
How far is it from Ballantyne?
About 30 miles, about 40 minutes by road, and the Google Maps link above gives you the live route and traffic. Truck parking is on site.
I drive a charter coach, not a truck. Is it the same exam?
Yes. The FMCSA medical certificate covers every commercial driver, passenger or freight, and the examination is the same. Bring your CDL with the passenger endorsement and the usual paperwork. Our charter and tour bus page covers what coach operators ask about most.
What does it cost?
$79 cash or $81.77 by card, and that is everything: the examination, the urine test, the certificate and the National Registry filing. If you were already booked at a quoted price, you pay the price you were quoted.
